Definitions

Unless otherwise specified, “EleosLife“, “we“, “us“ or “our“ refers to Eleos Life Insurance Services Incorporated and its affiliates. “Site“ refers to https://www.eleoslife.com/ (or any successor URL) and any other Internet sites, online services, or mobile applications operated by EleosLife. “You“ or “Your“ refers to the user of the Site.

Limitations of Use

The Site and the insurance products and services offered through it are intended solely for residents of the United States, excluding New York, who are at least 18 years of age and have reached the age of majority in their jurisdiction. EleosLife does not offer insurance products in New York.

You agree to use the Site only for lawful purposes and in compliance with all applicable federal, state, and local laws, regulations, and ordinances. Unauthorized use of the Site, including but not limited to unauthorized access to our systems, misuse of passwords, or misuse of information posted on the Site, is strictly prohibited.

We may modify the Site, discontinue offering products or services, or impose usage limits at any time without prior notice. We may also terminate or suspend your access to the Site without notice or liability for any lawful reason, including if we determine, in our sole discretion, that you have violated these Terms. Upon termination, you remain bound by these Terms with respect to any obligations incurred prior to termination. Any such changes or termination will not affect insurance policies you have already obtained through the Site.

Website Security

In the event of a security breach compromising data under our control, we will investigate, notify affected individuals as appropriate, and take further steps in accordance with applicable laws and regulations.

Insurance Products

EleosLife offers life and disability insurance products in all U.S. states except New York, subject to eligibility and the legal ability of our partner insurers to sell such products in your jurisdiction. These insurance products are underwritten and issued by third-party insurers, not EleosLife. We act as an intermediary and receive commissions from these insurers for policies sold through the Site.

1. Designate EleosLife as your agent of record;
2. Authorize EleosLife to communicate this designation to insurers, your prior insurance agents, or other relevant parties as we deem necessary; and
3. Permit EleosLife to receive commissions or other compensation from insurers related to your purchase.

Insurance product descriptions on the Site are for informational purposes only and do not constitute complete descriptions of terms, conditions, exclusions, or limitations. The terms of any issued policy will govern in the event of a conflict with these Terms or Site content. Approval of your application is not guaranteed and is subject to the underwriting guidelines and discretion of the issuing insurer. If you have a complaint about our services or products that we cannot resolve, you may contact your state’s insurance regulator (contact information available at www.naic.orghttps://www.naic.org).

Consent to Doing Business Electronically (Residents of the District of Columbia and All States Except New York)

To access the benefits of the insurance products and services offered through the Site, we require your consent to conduct business electronically. This section outlines your rights regarding electronic communications (“Communications“), which include disclosures, notices, applications, policies, and other documents related to your use of the Site or insurance products.

• Scope of Consent: You agree that all Communications may be provided electronically via email or the Site. You also authorize us to accept electronic signatures, which will have the same legal effect as physical signatures. This consent extends to Communications we send to you and information you provide to us.
• Hardware/Software Requirements: To access and retain Communications, you must have:
  • A device with a current internet browser;
  • Internet access and an email account;
  • The ability to view and print documents (e.g., via Adobe Reader); and
  • The ability to send and receive emails with hyperlinks. If using a mobile device, you must be able to print or save Communications or access the Site via a device with such capabilities.
• Withdrawing Consent: You may withdraw your consent to electronic Communications at any time by emailing us at help@eleoslife.com or mailing Eleos Life Insurance Services Incorporated, 8th Green, Dover, DE 19901, Attn: Legal. Withdrawal will prevent you from applying for new products through the Site and may halt processing of pending applications, though existing policies remain unaffected.

Your Obligations and Authorizations

You may browse the Site without registering, but to access certain features or apply for insurance, you must create an account with accurate and complete information, which you agree to keep updated. You are responsible for all activity on your account and must safeguard your password, notifying us immediately at help@eleoslife.com of any unauthorized use or security breach.

To provide our services, we may need to collect and manage your personal information, health data, or other materials (“Your Data“). You grant EleosLife a non-exclusive, royalty-free license to use Your Data solely to facilitate the insurance products and services offered through the Site. You represent that Your Data is accurate, complete, and provided without obligation on our part, and you authorize us to store and use it in accordance with applicable laws.

Prohibited Activities

You agree not to:

• Register accounts for others or impersonate anyone;
• Use another’s account without authorization;
